Senior MDT Co-ordinator

A fantastic new job opportunity has arisen for a dedicated Senior MDT Co-ordinator with a base in the Sittingbourne, Kent area. You will be working for one of UK’s leading health care providers

This service focuses on providing holistic assessment, planning delivery and evaluation of care to patients within their own homes

**You should be a Registered Nurse, Physiotherapist, Occupational Therapist or professional with relevant experience as well as a current registration with NMC or HCPC or relevant professional body**

As the Senior MDT Co-ordinator your key duties include:
• You will work with GPs, Health and Social Care professionals as part of a Multi-Disciplinary Team (MDT) approach
• To support the MDTs to signpost people to services that assist them to remain safely in their home and maximise their independence and physical, emotional and social well-being
• You will work across primary, secondary and community teams to utilise risk stratification tools (such as eFI, Rockwood clinical Frailty Scale and mede-analytics) to best effect in identifying opportunities for preventative and self-management support to patients
• You will also be responsible for ensuring that patients are referred on to the most appropriate health, social and/or voluntary organisations in a timely manner and effective coordination of actions from MDT meetings
• Working within the Long-term Conditions Services, you will be the clinical lead for the MDT Coordinator service

The following skills and experience would be preferred and beneficial for the role:
• You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, and Excel
• Excellent people skills and communication skills
• Have a positive approach to change
• Ensure high standards of service are maintained
