Rapid Response Team Sister

An amazing new job opportunity has arisen for a committed Rapid Response Team Sister with a base in a hospital in the Gravesend, Kent area. You will be working for one of UK’s leading health care providers

This hospital provides many services and works as a walk-in centre for patients who have an urgent but not life threatening illness or injury

**To be considered for this position you must be qualified as a Nurse either RGN, RMN or RNLD with a current active NMC Pin**

As the Rapid Response Team Sister your key duties include:
• Manage referrals and caseload, for a 7 day per week Rapid Response pathway
• Assist the Team Leader in running a team, deputizing for the Team Leader in their absence
• Actively work as a member of the Rapid Response Pathway to provide high quality nursing care to clients within a community setting, preventing unnecessary hospital admissions and supporting hospital discharges
• Be professionally accountable for the appropriate allocation of patient care to staff within the team and for the supervision of their work ensuring that it meets professional and clinical standards/guidelines
• Work with the Team Leader/Operational Lead to develop/audit the work of the team including Essence of Care Standards, Standards for Better Health Care, National Service Frameworks and long-term conditions management
• Be responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes
• Implement, monitor and maintain high standards of care delivery at all times.
• Care for patients and their families in the way that you believe is best
• Deliver our high service standards, sharing best practice, and actively improving the way we work

The following skills and experience would be preferred and beneficial for the role:
• Ability to work both independently and also as part of a wider multidisciplinary team
• Have strong communication and organisation skills and a flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey
• You will have a full UK driving license with access to a car for work purposes
• You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, and Excel
• Able to show can-do attitude always
